Business Requirements:

Amco, a supplier of medical grade mats for various industries, required an online form where users could easily find the mat they required for their needs while also supplying Amco with information about the client’s work environment, to ensure the right mat was suggested to them.

My Responsibilities:

Due to the mats having very specific purposes, Amco needed to ensure the user was getting exactly the right product for their needs, while also gather information about the user in order to market appropriately to them in the future.

My role was to design a form that would gather all this information, but be simple and engaging enough for the user to want to proceed through every step.

My Approach:

Amco supplied a summary of their mats and their purpose, and from there I filtered through all the information in an easy, concise format that was simple for both the client and their user.

I decided to identify situations where some questions need only be asked in certain circumstances (e.g. if no, then continue on, if yes, answer the below). For the case of the latter, I chose to have those questions only appear if they needed to as seen in the information architecture below. I ensured all questions were separated into no more than three steps, with descriptions in each step letting the user quickly know the nature of the section.

Impact:

Below is the information architecture for these forms, displaying and hiding information as required in order not to look too tedious for the user to fill in. Users were able to easily fill in the form and the client was able to receive the information they needed to send the user the right product and information.
